Who We Are

We are Geriatric Care Managers. As elder care specialists, we assess seniors in their home or facility and determine a plan of action to help them receive the care they need. We create and implement a care plan according to the responsible parties' wishes. We are advocates. We help sort through options. We act as liaison with physicians, attorneys, home health agencies, home care agencies, care facilities, hospice, etc.

Our experience in the field of geriatric care management helps us refer you to appropriate professionals in the legal, financial, and medical fields. Our broad based knowledge allows us to custom tailor a program to fit each client's specific needs.

We serve greater Los Angeles, Ventura and Orange County.

 

What We Do

We recognize that every family situation has its unique complexities and requires a custom fit solution. As your Geriatric Care Manager we will:

Assess the client's medical, psycho/social and living conditions to identify the care and safety needs Create a customized care plan designed to improve the client's overall physical, mental and emotional health while working within the family's budget.

Serve as liaison and advocate between the physician and family to make certain the client's healthcare needs are met.

Implement the care plan by making appropriate referrals to physicians, attorneys, home health agencies, care companion agencies, assisted living and board and care facilities, day programs and hospice when needed.

Contact appropriate care companion agencies with client criteria and interview companions to find a match that best fits the client's needs and behaviors.

Visit clients onsite to oversee and support their continued care and support home care workers.

Coordinate the entire process and update families through emails and phone calls.

Facilitate family meetings in a neutral and unbiased manner.

We are happy to do as much or as little as needed. We are private pay. We do not accept Medicare.
